The American Diabetes Association suggests that a Glucose Load Test be performed at 24 – 28 weeks of gestation in women not previously diagnosed with diabetes.
When conducting the two-step approach, the 50-gram glucose beverage should be consumed within 5 minutes. A plasma glucose measurement is then be performed at 1-hour. This test is performed at your laboratory or physician's office. Consult with your healthcare provider for more details. Please note, the 50-gram, 75-gram & 100-gram glucose tests have different requirements.

It’s important to ask your provider if you’ll be taking the 50-gram glucose load test (commonly known as the 1-hour test) or the 75-gram oral glucose tolerance test (commonly known as the 2-hour confirmatory test). The requirement varies between offices. Here are general guidelines as published in theAmerican Diabetes Association's 2022 Standards of Medical Care in Diabetes Journal.
If you’re taking the 50-gram glucose load test, the ADA suggests that this is a non-fasting test. You will be asked to consume a 50-gram glucose beverage (within 5 minutes) 1-hour prior to a plasma blood glucose draw. This will be performed at a laboratory or at your OBGYN’s office. If the plasma glucose value exceeds a specific threshold you will then be scheduled for a 100-gram oral glucose tolerance test. If you failed the 50-gram test and are required to do the 100-gram 3-hour glucose tolerance test, the ADA suggests a fasting plasma glucose measurement prior to consuming the beverage. Per your provider and laboratory instruction, subsequent glucose measurements will be taken at the 1-hour, 2-hour, and 3-hour mark.
If you’re taking the 75-gram oral glucose tolerance test, the ADA suggests that this is a fasting test. First, your plasma glucose will be measured. Then you will be asked to consume a 75-gram glucose beverage (within 5 minutes). Your plasma blood glucose will be measured again at 1-hour and 2-hour.
